Job Title: Registered Nurse - Home Health

Pinnacle Home Care Inc. is seeking a skilled Registered Nurse to join our team as a Home Health Nurse. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and education in the comfort of patients' homes.

Job Summary

The Registered Nurse will plan, organize, and direct home care services, utilizing their expertise in nursing and community health education. You will work closely with patients, families, and healthcare professionals to develop and implement personalized care plans that meet the unique needs of each individual.

Key Responsibilities
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
  • Provide direct patient care, including administering medications, wound care, and IV therapy, as prescribed by physicians.
  • Collaborate with healthcare teams to coordinate patient care, ensuring seamless transitions and optimal outcomes.
  • Communicate effectively with patients, families, and healthcare professionals, providing education and support to promote healthy behaviors and self-management.
  • Monitor patient progress, making adjustments to care plans as needed to ensure optimal health outcomes.
  • Supervise and evaluate home health aides, ensuring they provide high-quality care and support to patients.
Requirements
  • Graduate of an accredited school of nursing with a current RN license in the state.
  • One to two years of recent acute care experience in an institutional setting, with a preference for home health care experience.
  • CPR certification and a valid driver's license with a reliable vehicle.
  • Excellent observation, verbal, and written communication skills, with the ability to work independently and as part of a team.
