Job Title: Registered Nurse - Home Health

Pinnacle Home Care Inc. is seeking a skilled Registered Nurse to join our team as a Home Health Nurse. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and education in the comfort of patients' homes.

Job Summary:

The Registered Nurse will plan, organize, and direct home care services, utilizing their expertise in nursing and community health education. This role requires a strong ability to assess patient needs, develop individualized care plans, and coordinate with healthcare professionals to ensure seamless care delivery.

Key Responsibilities:
  • Conduct thorough patient assessments to identify care needs and develop personalized care plans.
  • Provide education and support to patients and their families on health management, disease prevention, and wellness.
  • Collaborate with healthcare professionals to ensure comprehensive care delivery and coordinate services as needed.
  • Monitor patient progress, adjust care plans as necessary, and communicate effectively with patients, families, and healthcare teams.
  • Supervise and evaluate home health aides to ensure high-quality care delivery.
Requirements:
  • Graduate of an accredited school of nursing with a current RN license in the state.
  • One to two years of recent acute care experience in an institutional setting, with home health care experience preferred.
  • CPR certification and a valid driver's license with a reliable vehicle.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, and basic math skills.
