Job Title: Registered Nurse - Home Health

Pinnacle Home Care Inc. is seeking a skilled Registered Nurse to join our team as a Home Health Nurse. As a Home Health Nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Job Summary:

The Registered Nurse plans, organizes, and directs home care services, with a focus on community health education and experience. The ideal candidate will have excellent observation, verbal, and written communication skills, as well as problem-solving skills and basic math skills.

Responsibilities:
  • Conduct initial assessments of patients and families to determine home care needs.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide direct patient care, including administering medications and treatments as prescribed by physicians.
  • Counsel patients and families on meeting nursing and related needs.
  • Collaborate with community health-related persons to coordinate care plans.
  • Participate in on-call duties as defined by the on-call policy.
Requirements:
  • Graduate of an accredited school of nursing.
  • One to two years of recent acute care experience in an institutional setting.
  • Home health care experience preferred.
  • Current licensure in state and CPR certification.
