Home Health Nurse

Pinnacle Home Care Inc. is seeking a skilled Registered Nurse to join our team as a Home Health Nurse. As a Home Health Nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes.
Job SummaryThe Registered Nurse will plan, organize, and direct home care services, with a focus on community health education and experience. You will work closely with patients and their families to develop personalized care plans, provide education and support, and coordinate with healthcare providers to ensure seamless care.
Responsibilities- Conduct initial assessments of patients and families to determine home care needs, providing a comprehensive physical assessment and history of current and previous illnesses.
- Regularly re-evaluate patient nursing needs and make necessary revisions to the care plan.
- Develop and implement individualized care plans, incorporating therapeutic, preventative, and rehabilitative nursing actions.
- Administer medications and treatments as prescribed by physicians, provide direct patient care, and counsel patients and families on meeting nursing and related needs.
- Identify discharge planning needs and implement prior to discharge, acting as a Case Manager when assigned by Clinical Supervisor.
- Prepare clinical notes and update primary physicians as necessary, communicating with community health-related persons to coordinate care plans.
- Participate in on-call duties and ensure arrangements for equipment and other necessary items and services are available.
To be successful in this role, you will need:
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Graduate of an accredited school of nursing, with one to two years of recent acute care experience in an institutional setting, and home health care experience preferred.
- Current licensure in the state and CPR certification.
- Management experience is not required, but you will be responsible for supervising LPNs and home health aides.
