LPN Care Coordinator

EngageMED, Inc. is seeking a skilled LPN to fill the role of Care Coordinator. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring seamless transitions between healthcare settings.
Key Responsibilities- Collaborate with healthcare providers and other team members to identify patient populations and assess their health care, educational, and psychosocial needs.
- Develop and implement individualized plans of care, working closely with patients, families, and caregivers to ensure optimal health and independence.
- Provide patient self-management support, empowering patients to achieve their health goals and manage chronic conditions.
- Facilitate clinical interventions based on protocols and evidence-based guidelines, fostering a team approach and involving patients and their families as active members of the team.
- Complete timely follow-up with patients discharged from the ED or Inpatient setting, reviewing discharge instructions and medications, scheduling follow-up appointments, and coordinating necessary services.
- Demonstrate critical thinking skills and positive relationship-building communication skills, both written and verbal.
- Maintain required documentation for all patient outreach and care coordination activities.
- Demonstrate customer-focused interpersonal and communication skills to interact effectively with healthcare providers, patients, and families.
- Demonstrate ability to work autonomously and be directly accountable for assigned duties.
- Demonstrate time management, priority setting, and work organization skills.
- Demonstrate ability to function effectively in a fluid, dynamic, and rapidly changing environment.
- Current Medical Assistant Certification and/or Registration or Current LPN license.
- Previous care of patients in primary/ambulatory care or other healthcare facilities, in the past five years.
- Critical thinking skills and ability to analyze data.
- Excellent verbal and written communication skills.
- Excellent interpersonal and facilitation skills.
- Ability to affect change, work as a productive and effective team member, and adapt to changing needs/priorities.
- Demonstrates excellent time management, priority setting, work delegation, and work organization skills.
General computer knowledge and capability to use various computer programs.
