Job Summary

We are seeking a dedicated and compassionate Care Coordinator to join our team at Arkansas Total Care. As a Care Coordinator, you will play a vital role in supporting our members by providing personalized care management services and ensuring seamless transitions between healthcare providers.

Key Responsibilities:

  • Provide outreach and engagement to members through phone calls and home visits to discuss care plans and services
  • Coordinate care activities based on individualized care plans and collaborate with healthcare providers and community organizations
  • Support members and caregivers with education and resources to promote healthy outcomes
  • Document and maintain accurate member records in accordance with regulatory requirements
  • Collaborate with care managers, practitioners, and other stakeholders to ensure continuity of care

Requirements:

  • High School diploma or equivalent required
  • 1-2 years of experience in a related field preferred
