Job Title: LPN Care Coordinator

We are seeking a skilled LPN to join our team at Pro Care Innovations. As a LPN Care Coordinator, you will play a vital role in providing high-quality care to our patients through our telehealth platform.

Key Responsibilities:
  • Provide at least 20 minutes of care coordination for Medicare patients each month via our telehealth platform.
  • Conduct telephone interactions with patients to assess their needs and develop personalized care plans.
  • Document patient interactions and chart reviews to ensure accurate and up-to-date information.
  • Collaborate with healthcare providers and other facilities to coordinate patient care and optimize outcomes.
  • Utilize strong communication and interpersonal skills to build trust with patients and providers.
Requirements:
  • At least 2 years of healthcare experience, preferably with Electronic Medical Records (EMRs).
  • Self-starter with excellent patient assessment skills and a strong understanding of common medical terminology.
  • Strong computer literacy and multitasking skills, including the ability to navigate multiple windows and screens quickly.
  • Excellent verbal and written communication skills, with the ability to effectively communicate with patients, providers, and colleagues.
  • Comfortable with outbound calling and enrolling patients into Medicare programs.
