Job Summary and Qualifications
  • This is a Full-Time Day Shift position, typically from 8:00 am to 4:30 pm, Monday through Friday, with rotating weekends.
  • Our RN Case Managers set the standard by delivering clinical expertise and the highest quality care in a compassionate manner.

The RN Case Manager is responsible for advocating patient-centered care by coordinating the care plan during the patient's stay, managing the length of stay, ensuring appropriate resource management, and developing a safe and suitable discharge plan in collaboration with the multidisciplinary team.

The RN Case Manager facilitates the progression and transition of care using established criteria in conjunction with the multidisciplinary team.

The RN Case Manager will coordinate activities that promote quality outcomes and patient throughput while balancing optimal care with appropriate resource utilization.


What You Will Do In This Role:

Conduct a comprehensive assessment of psychosocial, medical, and discharge needs of patients/families, along with an evaluation of resources available to the patient/family. Reassess the patient's clinical condition as necessary.

Consider the patient's readmission status or risk of readmission and develop strategies to mitigate this risk, including education on accessing healthcare resources, preventative education, and community-based resources.

Coordinate the care plan and drive the discharge process by collaborating with the multidisciplinary healthcare team, particularly with the patient's physician, to facilitate a successful care transition. In partnership with Social Services, the RN CM ensures that post-acute medical needs and level of care are appropriate.

The RN CM is responsible for timely referrals to Social Services when psychosocial determinants of health are identified. Involve the patient, family, and significant others in identifying and clarifying needs and expectations to establish mutual and realistic goals.

Evaluate the progression of care using evidence-based tools and approved criteria (InterQual) throughout the care episode; escalate care progression and transition issues through the established chain of command.

Make appropriate referrals to third-party payers, disease, and case management programs for recurring patients and those with chronic conditions. Facilitate patient throughput with a continuous focus on effective care transitions, quality, and efficiency.

Document professional recommendations, discharge plans, care coordination interventions, and case management activities to effectively communicate with all members of the healthcare team. Align patient needs with available resources to ensure a safe discharge/transition.

Act as a liaison through effective and professional communication between physicians, patients/families, hospital staff, and external agencies. Direct activities to identify and address the needs of under-resourced patient populations, including patient education activities, patient assistance programs, and community-based resources.

Demonstrate knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives. Serve as an advocate for patients' rights, needs, and values, ensuring that patients' ethnic, cultural, or religious values, beliefs, preferences, and needs are considered and respected.

What qualifications you will need:
Registered Nurse with a current TX state license required. Graduate of an accredited nursing school, Associate's Degree required, Bachelor's Degree preferred. 3+ years of clinical hospital nursing experience required, with 2 years of Case Management experience preferred. 2 years of critical care experience preferred. Certification in case management preferred. InterQual experience preferred.
