POSITION SUMMARY

The Community Outreach Support Specialist (COSS) is a field-oriented role dedicated to delivering culturally sensitive outreach and engagement services aimed at fostering connections with individuals facing homelessness. Utilizing a compassionate, person-centered methodology, the COSS strives to alleviate homelessness by offering tailored services that meet the unique needs of each individual. This role includes linking individuals to essential services that address the root causes of homelessness, such as substance use disorder (SUD) treatment, mental health support, and housing assistance. The COSS will provide continuous support and follow-up services throughout the individual's treatment journey to ensure sustained engagement.

KEY RESPONSIBILITIES

  • Utilizes personal experiences and professional expertise to establish rapport and trust with clients.
  • Collaborates with Homeless Outreach Teams to initiate contact with individuals experiencing homelessness and facilitate access to available resources.
  • Conducts outreach through the facilitation of SUD groups and/or individual meetings at various offsite community programs.
  • Maintains ongoing contact and develops relationships with individuals in the precontemplation stage of change.
  • Educates clients on overdose prevention and reversal techniques.
  • Engages in harm reduction initiatives in collaboration with local healthcare programs and outreach teams.
  • Administers Community Assessment Tool (CAT) and inputs data into necessary systems as required.
  • Facilitates timely entry into detoxification and residential treatment, including coordination with healthcare providers for medical clearance and Medication Assisted Treatment services.
  • Assists individuals in navigating enrollment processes for available community resources.
  • Builds and sustains effective partnerships with substance use disorder, mental health, and medical service providers.
  • Keeps accurate records and files, preparing reports as necessary.
  • Monitors adherence to grant and contract stipulations.
  • Ensures the confidentiality of client information and health records.
  • Safely transports clients while adhering to traffic laws and regulations.

QUALIFICATIONS

  • High school diploma or equivalent required.
  • Minimum of 1 year of experience in recovery processes related to mental illness or substance use disorder, either as a consumer or as a caregiver.
  • If not already certified as a Medi-Cal Peer Support Specialist, must enroll in an approved certification program within 60 days of employment.
  • Must possess and maintain a valid driver's license with a clean driving record for insurability.
