Job Overview

Position Title: Case Manager – Supportive Services

Classification: Non-Exempt / Full-Time (40 hours per week)

POSITION SUMMARY: Established in 1979, Interfaith Community Services is dedicated to addressing the increasing challenges of hunger and homelessness in North San Diego County. Our mission is to empower individuals to overcome poverty and homelessness through comprehensive support services. As the leading social services organization in North County, we provide immediate assistance and long-term programs that cater to the holistic needs of those in crisis. In collaboration with over 250 diverse faith communities, more than 650 dedicated volunteers, and a growing team of over 200 staff members, we serve over 17,000 individuals each year, offering a safe environment and a pathway to self-sufficiency.

KEY RESPONSIBILITIES:

  • Deliver direct client services, including intake, assessment, case management, and advocacy for homeless and low-income individuals, aiming to transition clients from crisis to self-sufficiency.
  • Assist clients in recognizing their strengths, establishing realistic goals, and maintaining consistent progress towards self-sufficiency.
  • Implement Housing First and Diversion strategies to support clients facing or at risk of homelessness.
  • Administer supportive services in accordance with agency and contractual guidelines.
  • Identify resources and advocate for the needs of clients.
  • Connect clients with resources within the agency and collaborate with other public and private social service organizations as needed.
  • Work independently and professionally at partner agencies, faith centers, and various community settings.
  • Collaborate effectively with volunteers, interns, and other agency personnel.
  • Provide services in diverse community settings, including other agency programs, faith centers, and educational institutions.
  • Meet specific monthly outcome requirements as outlined by program contracts.
  • Maintain accurate data entry in multiple databases and tracking systems.
  • Participate in all staff meetings and training sessions.
  • Perform additional responsibilities as assigned.

QUALIFICATIONS:

  • Bachelor's Degree in Social Services or a related field, or an equivalent combination of education and experience.
  • A minimum of 1 year of relevant work experience.
  • Bilingual proficiency in Spanish and English is preferred.
