Community Care Coordinator

Position Overview:
As a Community Care Coordinator at Complete Care Management, you will be instrumental in facilitating seamless transitions for patients moving from hospitals to our post-acute care facilities. Your commitment and empathy will significantly enhance the quality of life for our residents, ensuring they receive the outstanding care they require.
Key Responsibilities:
- Evaluate patients being discharged from hospitals to ascertain their care requirements in a post-acute environment.
- Work collaboratively with hospital personnel, patients, and their families to create detailed transition plans.
- Ensure effective communication between hospital staff, facility personnel, patients, and families.
- Provide education to patients and their families regarding care processes.
- Identify and pursue opportunities to increase market share from designated hospital accounts.
- Exhibit strong organizational skills to manage multiple tasks efficiently.
- Guarantee prompt communication and acceptance of hospital referrals.
Qualifications:
- A Bachelor's degree in healthcare administration, business, or a related discipline is preferred.
- 3-5 years of experience in roles such as Admissions Director or Hospital Liaison within skilled nursing facilities or acute rehabilitation hospitals.
- Excellent communication and interpersonal abilities.
- Familiarity with healthcare regulations and standards.
