Position Overview:
As an External Care Coordinator with Complete Care Management, you will be instrumental in facilitating seamless transitions for patients moving from hospitals to our post-acute care facilities. Your commitment and empathy will significantly influence the lives of our residents, guaranteeing they receive the exceptional care they deserve.
Key Responsibilities:
- Evaluate patients being discharged from hospitals to assess their care requirements in the post-acute environment.
- Work collaboratively with hospital personnel, patients, and families to create comprehensive transition strategies.
- Ensure effective communication between hospital and facility staff, patients, and their families.
- Provide education to patients and their families regarding care options.
- Identify opportunities to enhance market share from designated hospital accounts.
- Exhibit strong organizational skills to manage multiple tasks efficiently.
- Facilitate timely communication and acceptance of hospital referrals.
Qualifications:
- A Bachelor’s degree in healthcare administration, business, or a related discipline is preferred.
- 3-5 years of experience in roles such as Admissions Director or Hospital Liaison within skilled nursing or rehabilitation settings.
- Excellent communication and interpersonal abilities.
- Familiarity with healthcare regulations and standards.
