About the Position

We are seeking a dedicated Case Management Specialist to join our Foster Care program. This role is crucial in supporting families and children within our care, focusing on permanency planning and creating nurturing environments for growth and well-being.

Responsibilities
  • Develop comprehensive service plans in collaboration with families, addressing safety, behavioral, educational, psychological, and permanency needs.
  • Monitor the safety, well-being, and progress of children and families through regular casework contacts and meticulous documentation.
  • Foster strong communication and collaboration with internal and external service providers to ensure coordinated support for families.
  • Actively participate in Family Court Hearings, Family Team Conferences, and other meetings to advocate for the best interests of children and families.
Qualifications
  • Bachelor's degree in Social Work, Sociology, Psychology, or a related field. Bilingualism in Spanish is highly preferred.
  • Excellent verbal and written communication skills, time management abilities, and organizational skills.
  • Proficiency in computer applications, including MS Office and NYS Connections.
  • A genuine commitment to working with diverse populations.
