About This Role

CenterWell Home Health is seeking a skilled Speech Language Pathologist to join our team. As a Speech Language Pathologist, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Key Responsibilities

  • Evaluate, direct, and provide speech/language pathology services to patients in a home or facility setting, ensuring effective communication and swallowing disorders are addressed.
  • Participate in the development and periodic review of the Plan of Treatment and Plan of Care, collaborating with interdisciplinary teams to ensure comprehensive patient care.
  • Administer and interpret diagnostic tests and therapeutic treatments, including audio logic screening, to accurately assess and address patient needs.
  • Provide instruction and training to patients in the use of alternative communication systems when appropriate, promoting independence and self-advocacy.
  • Maintain and submit required documentation and reports, adhering to regulatory standards and organizational policies.
  • Participate in care coordination activities and discharge planning, ensuring seamless transitions and optimal patient outcomes.

Requirements

  • Certification of Clinical Competence in Speech Language Pathology or Audiology, demonstrating expertise and commitment to the field.
  • Minimum of six months experience as a speech therapist/speech-language pathologist, showcasing a strong foundation in patient care and communication.
  • Current and unrestricted license, ensuring compliance with regulatory requirements.
  • Current CPR certification, demonstrating a commitment to patient safety and well-being.

A valid driver's license, auto insurance, and reliable transportation are required for this role, enabling you to effectively travel to patient locations and provide high-quality care.
