We are seeking a skilled Physical Therapist to join our team at CenterWell Home Health. As a Physical Therapist, you will play a vital role in helping our patients recover from injuries, illnesses, and functional disabilities.
Key Responsibilities- Plan and administer prescribed skilled physical therapy treatment and training for patients to attain the highest level of physical function.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
- Develop and implement conditioning and rehabilitation programs consistent with physicians' plans of treatment and the overall goals of the patient/rehab team.
- Adjust treatment as needed to achieve maximum results and confer with physicians and clinical team members to obtain additional patient information.
- Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aide staff, supervise and evaluate their performance, and monitor the completion of documentation.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations, and assure that interim orders are promptly and accurately documented and implemented.
- Report patients' progress to physicians, Clinical Managers, staff, patients, and families, and submit evaluation, treatment plans, progress reports, and discharge summaries to supervisors and care management staff.
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
