About the Role

We are seeking a highly skilled Physical Therapist to join our team at CenterWell Home Health. As a Physical Therapist, you will play a critical role in helping our patients recover from injury, illness, or surgery, and regain their independence.

Key Responsibilities
  • Develop and implement personalized physical therapy treatment plans for patients in their homes, tailored to their specific needs and goals.
  • Conduct thorough assessments of patients' physical abilities and create treatment plans to address any deficits or limitations.
  • Provide one-on-one therapy sessions to patients, using a variety of techniques and modalities to promote healing and recovery.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care for patients.
  • Document patient progress and outcomes, and communicate with patients, families, and healthcare teams to ensure seamless care.
  • Stay up-to-date with the latest research and best practices in physical therapy, and incorporate this knowledge into patient care.
Requirements
  • Master's degree in Physical Therapy from an accredited program.
  • Current licensure as a Physical Therapist in the state of practice.
  • Minimum of one year of experience in home health or similar setting.
  • Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.
