Job Description

Job Summary:

We are seeking a highly motivated and organized Service Coordinator to join our team at Advantage Home Care. As a Service Coordinator, you will play a critical role in ensuring the smooth delivery of patient care services to our clients.

Key Responsibilities:

  • Client Communication: Develop and maintain positive relationships with clients, their families, and healthcare providers to ensure seamless care coordination.
  • Scheduling and Coordination: Schedule and coordinate patient care services, including home health care, skilled nursing, and therapy services.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date records of client interactions, care plans, and service delivery.
  • Team Collaboration: Work closely with our care team to ensure that client needs are met and that care plans are implemented effectively.
  • Problem-Solving and Conflict Resolution: Identify and resolve issues that may impact client care, such as scheduling conflicts or service delivery problems.

Requirements:

  • Education: High school graduate or equivalent required.
  • Experience: Minimum of 2 years of experience in a healthcare or customer service role.
  • Skills: Excellent communication and organizational skills, with the ability to work in a fast-paced environment.
