The Community Health Worker plays a vital role in delivering hands-on support to individuals experiencing various levels of mental health challenges and substance use disorders. KEY RESPONSIBILITIES:
- Complete progress notes that document service delivery in the electronic medical record (EMR) system promptly and in line with the standards set by the Program Director, while also noting and reporting any symptoms requiring immediate professional intervention.
- Facilitate educational, recreational, and therapeutic activities for clients, including organizing office events.
- Maintain comprehensive documentation of clients' behaviors and their mental, behavioral, medical, and substance use histories.
- Create a safe and supportive environment for each client, ensuring a caring atmosphere.
- Exhibit strong interpersonal and observational skills.
- Assist in managing clients' medications and monitoring vital signs.
- Act as the primary point of contact for delivering supportive mental and behavioral healthcare and connecting clients with helpful resources.
- Comply with HIPAA regulations.
ESSENTIAL QUALIFICATIONS:
- Excellent listening, verbal, and written communication abilities.
- Proficient in Microsoft Office Suite, including Word, Excel, Outlook, PowerPoint, as well as familiarity with Office 365, Teams, Zoom, and other communication platforms.
- Basic understanding of California benefits navigation.
- Ability to stand and/or walk for extended periods.
REQUIRED CERTIFICATIONS:
- High school diploma or GED.
- Strong communication capabilities.
- CPR/First Aid and Food Handlers Certification (to be obtained prior to employment).
- Current TB test and physical examination (to be completed prior to employment).
- Experience in mental health and substance use treatment.
- Current RADT certification.
