JOB SUMMARY

The Practical Nursing Specialist is a graduate of an accredited nursing program and holds a valid license to deliver safe and proficient patient care under the guidance of a Physician and/or Registered Nurse. This role emphasizes evidence-based practices in accordance with the Nurse Practice Act and other regulatory bodies, showcasing strong leadership and effective skills in managing patients with chronic conditions. The Practical Nursing Specialist collaborates closely with advanced practice clinicians and benefits from constructive feedback, reporting directly to the Director of Nursing.

ESSENTIAL FUNCTIONS

  1. Assess all patients presenting with medical emergencies to determine their condition and coordinate follow-up with medical Providers.
  2. Evaluate patients scheduled for routine appointments.
  3. Oversee daily clinical patient flow, ensuring safety and addressing any challenges within the clinical team.
  4. Travel between locations as needed to provide direct patient care, triage, and other assigned responsibilities.
  5. Conduct health and educational assessments during patient intake and manage all aspects of the initial triage process.
  6. Act as a patient advocate and consult with physicians as necessary.
  7. Assist in coordinating resources with the interdisciplinary team.
  8. Support physicians in delivering individual health education and provide relevant literature.
  9. Schedule appointments for physicians and Pharmaceutical Representatives.
  10. Contribute to the development of policies, procedures, and clinical protocols.
  11. Participate in case conferences with the interdisciplinary team, providing necessary supporting information.
  12. Conduct monthly quality assurance checks on all assigned medical equipment.
  13. Maintain appropriate inventory levels of injectable medications and administer as required.
  14. Order medical supplies as needed.
  15. Procure office supplies for the medical team.
  16. Prepare monthly, quarterly, and annual reports in a timely manner.
  17. Encourage professional growth among other Licensed Practical Nurses, Medical Assistants, Laboratory Technicians, and other team members.
  18. Foster positive, professional relationships within and outside the agency.
  19. Assist in developing orientation programs for new employees and serve as a nurse preceptor.
  20. Complete the monthly clinical schedule and submit it to the Nurse Manager as directed.
  21. Support the Director of Nursing with administrative tasks as assigned.
  22. Empower clients to make informed health decisions to enhance their quality of life.
  23. Maintain patient records in compliance with the Health Insurance Portability and Accountability Act (HIPAA).
  24. Process routine, urgent, and Stat referrals.
  25. Understand insurance processes and requirements.
  26. Attend team meetings and provide clinical updates as necessary.
  27. Obtain Mediquire reports in advance to prepare for daily huddles and identify gaps in care.
  28. Perform other duties as assigned.

EDUCATION AND EXPERIENCE
  • Must possess a valid license to practice in the state of Virginia.
  • Current CPR (Healthcare Provider) certification is required.
  • A minimum of three years of nursing experience is necessary.
  • A valid Virginia Driver's License, satisfactory driving record, and reliable transportation for travel to various medical offices are required.
