Job Overview

Under the guidance of the Program Director, the Case Manager plays a pivotal role in delivering comprehensive case management and housing assistance to participants. This position also involves fulfilling monthly reporting obligations to ensure adherence to program standards. The qualifications outlined below reflect the necessary knowledge, skills, and abilities required for this role. PRIMARY RESPONSIBILITIES:

Housing Coordination

  • Serves as the primary contact between participants, building managers, and superintendents to guarantee that all maintenance requests are addressed promptly.
  • Maintains a detailed log of building and apartment conditions, including dates and status updates.
  • Reports any compliance issues regarding apartments and buildings to the Supportive Housing Manager, particularly those requiring urgent attention.

Case Management and Documentation

  • Provides essential case management and crisis intervention services on-site.
  • Completes necessary service plans for participants.
  • Facilitates monthly tenant meetings to foster community engagement.
  • Educates participants on effective housekeeping, home safety, budgeting, and landlord-tenant relationships as needed.
  • Keeps thorough records of all interactions with tenants.
  • Documents monthly progress notes in participants' files.
  • Conducts home visits to assess compliance with program rules and identify needs for additional support services.
  • Collects random urine samples for drug screening purposes.
  • May accompany participants to social service agencies upon request.
  • Reports any suspected child neglect or abuse to the Supportive Housing Manager.

Housing Compliance and Reporting

  • Responsible for preparing annual tenant rent calculation forms.
  • Completes annual apartment inspection documentation.
  • Updates the monthly program database to reflect current participant information.
  • Ensures that annual leases are properly updated and filed.

QUALIFICATIONS:

  • Associate's Degree or a minimum of 60 verifiable college credits; Bachelor's Degree preferred.
  • At least one year of relevant experience in a Mental or Behavioral Health setting is required.
  • Demonstrated ability to apply business judgment and make sound decisions.
  • Proficient in handling sensitive matters with discretion and diplomacy.
  • Strong organizational skills with the ability to manage multiple tasks effectively.
  • Excellent interpersonal communication skills, both verbal and written.
  • Commitment to promoting cultural diversity and sensitivity.
  • Ability to maintain professionalism under pressure while demonstrating a strong commitment to quality and urgency.
