The ideal candidate will have a strong background in working with individuals with special needs and a passion for providing person-centered care. As a Community Habilitation Specialist, you will work collaboratively with our team to develop and implement individualized service plans that promote independence, socialization, and community integration.

Responsibilities will include:

  • Providing training and support to individuals with developmental disabilities in areas such as daily living skills, socialization, and employment readiness.
  • Ensuring the safety and security of individuals receiving services, while also promoting their independence and autonomy.
  • Collaborating with our team to develop and implement individualized service plans that meet the unique needs and goals of each individual.
  • Providing timely and effective services that are tailored to the needs and preferences of each individual.
  • Documenting service delivery and progress towards individualized goals.

Requirements for this role include:

  • A high school diploma or equivalent.
  • At least three years of experience working with individuals with developmental disabilities.
  • Excellent interpersonal, verbal, and written communication skills.
  • Proficiency in Microsoft Word and other relevant software applications.
  • Ability to travel to other boroughs as assigned.
