Patient Transition Specialist

Norco, a family and employee-owned organization, is currently in search of a committed individual to fill the role of Patient Transition Specialist. In this capacity, you will be instrumental in facilitating patient discharges from healthcare facilities while fostering robust connections with medical professionals. Your key responsibilities will encompass:
- Engaging with discharge coordinators and serving as a communication bridge between Norco Medical and patients
- Supporting referral processes and ensuring seamless care transitions for patients
- Maintaining comprehensive knowledge of Norco's products and services
- Managing documentation and various administrative responsibilities
- Performing additional tasks as required
Compensation: $18-$21 per hour (Based on Experience)
Requirements:- Preferred: One year of experience in home care settings
- Preferred: Background in sales and marketing
- Capability to lift up to 50 lbs and perform basic mathematical calculations
- Excellent manual dexterity for handling documentation
