Position Overview
Norco is actively seeking a dedicated individual to assume the role of Home Transition Coordinator. This position plays a vital role in enhancing the efficiency of patient discharges from healthcare facilities, while also embodying Norco's commitment to excellence within the provider community. The Home Transition Coordinator serves as a key representative of customer service, ensuring a seamless transition for patients.
Key Responsibilities:
- Collaborate with discharge planners as an integral part of the Sales team.
- Serve as a communication bridge between Norco Medical and patients, their families, caregivers, physicians, and other healthcare professionals involved in the case.
- Support Customer Service Representatives in coordinating essential referrals to maintain continuity of care for the patient population.
- Maintain awareness of third-party reimbursement policies, service criteria, equipment offerings, billing procedures, and patient documentation.
- Exhibit comprehensive knowledge of products, including their placement, availability, pricing, and application.
- Perform additional duties as required or assigned.
- Compensation: $18-$21 per hour, depending on experience.
Qualifications:
- Preferred: One year of experience in home care.
- Preferred: Experience in sales and marketing.
- Ability to frequently lift up to 50 lbs and occasionally over 100 lbs.
- Proficient in basic mathematical calculations.
- Capable of performing tasks requiring manual dexterity, such as processing paperwork, filing, stapling, and sorting.
