Community Support Advocate

North Suffolk Community Services Inc is in search of compassionate professionals to enhance our Emergency Respite team. As a Community Support Advocate, your contribution will be vital in connecting with, evaluating, and devising treatment strategies for individuals requiring assistance. Your duties will encompass delivering direct support in various domains, including daily living skills, transportation, and medication oversight, all tailored to personalized treatment plans. The emphasis is on fostering the overall health and well-being of those you support.
Core Responsibilities:
- Engage with individuals to enhance their health and wellness, attain stability, and cultivate skills for independent living.
- Assist individuals in establishing objectives that resonate with their personal strengths and interests to facilitate their recovery process.
- Provide care services with empathy and adaptability.
- Implement strategies to mitigate and address potential risks effectively.
- Accurately and confidentially document individual progress.
- Deliver interventions and support services as specified in treatment plans.
Qualifications:
- Education:
- High School Diploma/GED is mandatory
- Bachelor's Degree is preferred
- Experience:
- At least one year of experience working with individuals facing mental health challenges.
- Skills/Other:
- Valid Driver's License
- MAP certification to be obtained within five months of employment
- CPR/First Aid certification to be acquired within five months of employment
