As a Community Health Advocate, you will work closely with patients living with complex medical and behavioral health conditions. You will guide them through their healthcare journey by:
- Conducting thorough screenings to identify patients' social needs and potential barriers to care.
- Determining eligibility for community programs and benefits, such as disability assistance, food stamps, and transportation services.
- Providing personalized support to patients facing challenges with paperwork or language barriers, ensuring they can access vital resources.
- Serving as a valuable resource for our clinical teams by sharing information about available community services and training them on how to utilize our internal database.
Your responsibilities will also include:
- Tracking patient interactions and identified needs within our electronic medical record system (EPIC).
- Collaborating with the clinical team to close care gaps by reviewing patient charts and identifying areas for improvement.
- Utilizing motivational interviewing techniques to empower patients and encourage them to actively participate in their healthcare.
- Educating patients about cancer screening options, addressing any concerns they may have, and assisting with scheduling appointments.
- Providing ongoing support to patients who undergo cancer screenings outside of our health center.
- Maintaining accurate records of patient interactions and outcomes within EPIC.
- Leveraging technology platforms like MyChart and texting to effectively communicate with patients.
- Monitoring the results of cancer screenings and initiating follow-up actions as needed.
- Participating in professional development opportunities and team trainings to enhance your knowledge of community resources and social justice issues in healthcare.
