Job Summary

2Life Communities is seeking a highly skilled Community Support Coordinator to join our team. As a key member of our staff, you will play a vital role in providing exceptional support services to our residents, ensuring they receive the care and assistance they need to thrive in their communities.

Key Responsibilities
  • Conduct Resident Assessments: Conduct regular assessments of residents' social, psychological, cognitive, and physical status, and develop personalized plans to address their needs.
  • Advocate for Residents: Advocate on behalf of residents to ensure they receive timely and cost-effective access to essential services.
  • Participate in Housing and Health Care Initiatives: Collaborate with healthcare providers and community organizations to develop and implement innovative solutions to support residents' overall health and well-being.
  • Respond to Resident Emergencies: Respond promptly to resident emergency calls, working closely with other staff members to ensure timely and effective support.
  • Educate and Assist Residents: Educate and assist residents and their families on available services and resources, both within 2Life Communities and in the broader community.
  • Pursue Additional Services: Identify and pursue opportunities for additional services through private, local, state, and federal sources.
  • Document Resident Interactions: Maintain accurate and confidential records of all significant interactions with residents, social service providers, medical providers, families, and for HUD reporting purposes.
  • Coordinate Care Transitions: Coordinate care transitions across medical providers, ensuring a safe and seamless transition for residents and their families.
Requirements
  • Education and Certification: Master's or Bachelor's degree in Social Work, Gerontology, or a related field, with Resident Services Coordinator Certification or willingness to attain within the first year of employment.
  • Experience and Skills: Proven experience working with older adults, general computer literacy with Microsoft Office products and database software, and bilingual proficiency in Russian.
