Delta-T Group is seeking a skilled Behavioral Health Specialist to join our team. The ideal candidate will have a Master's Degree in Social Work and 1-5 years of experience in the Human Services field, preferably working with adolescents and/or young adults. The successful candidate will be responsible for connecting with and working successfully alongside youth and families, ensuring client files and records are accurate and up-to-date, and participating in live supervision with clients and their families.
Key Responsibilities- Takes a position of leadership in supporting program structure and all youth within the program
- Demonstrates initiative in supporting and empowering the youth and families
- Ensures client files, records, monthly reviews, and Individual Service Plans are accurate, updated, and secure
- Participates in live supervision with clients and their families
- Meets regularly with the Program Manager for supervision
- Demonstrates engagement and leadership in all weekly staff meetings
- Assists clients with discharge and aftercare planning as they transition into their next step following the program
- Provides the Program Manager with timely and accurate documentation of all services provided, including client progress reports and reports of problems that require immediate attention
- Oversees and reviews client assessments, provides case management services, and oversees the development of individual treatment plans for clients and families
- Master's Degree in Social Work (MSW) Required
- 1-5 years of experience in the Human Services field, preferably working with adolescents and/or young adults
- Expert knowledge of issues surrounding homelessness
- Demonstrated ability to work with community leaders to raise funding for programs and skills to conceptualize and effectively lead complex projects
