Job Summary

Delta-T Group is seeking a skilled Behavioral Health Specialist to join our team. The ideal candidate will have a Master's Degree in Social Work and 1-5 years of experience in the Human Services field, preferably working with adolescents and/or young adults. The successful candidate will be responsible for connecting with and working successfully alongside youth and families, ensuring client files and records are accurate and up-to-date, and participating in live supervision with clients and their families.

Key Responsibilities
  • Takes a position of leadership in supporting program structure and all youth within the program
  • Demonstrates initiative in supporting and empowering the youth and families
  • Ensures client files, records, monthly reviews, and Individual Service Plans are accurate, updated, and secure
  • Participates in live supervision with clients and their families
  • Meets regularly with the Program Manager for supervision
  • Demonstrates engagement and leadership in all weekly staff meetings
  • Assists clients with discharge and aftercare planning as they transition into their next step following the program
  • Provides the Program Manager with timely and accurate documentation of all services provided, including client progress reports and reports of problems that require immediate attention
  • Oversees and reviews client assessments, provides case management services, and oversees the development of individual treatment plans for clients and families
Requirements
  • Master's Degree in Social Work (MSW) Required
  • 1-5 years of experience in the Human Services field, preferably working with adolescents and/or young adults
  • Expert knowledge of issues surrounding homelessness
  • Demonstrated ability to work with community leaders to raise funding for programs and skills to conceptualize and effectively lead complex projects
