Job Overview

The Referral Coordinator plays a crucial role in ensuring a seamless process for patient referrals within the healthcare system. This position involves various responsibilities aimed at enhancing patient care and operational efficiency.

Key Responsibilities:
  • Maintain comprehensive tracking and documentation of patient referrals to foster team collaboration and ensure safety.
  • Verify and update patient registration details, including demographics and insurance coverage.
  • Input and manage referral data within the electronic health record system as necessary.
  • Gather pertinent information regarding the patient's medical history and referral requirements.
  • Communicate with insurance providers and review organizations to confirm prior authorization protocols are fulfilled.
  • Provide essential medical details such as patient history, diagnosis, and prognosis.
  • Collaborate with financial services to ensure optimal reimbursement for healthcare services rendered.
  • Discuss referral details and expectations with patients to enhance understanding.
  • Assist patients in navigating potential challenges related to healthcare access, including financial or social barriers.
  • Identify and leverage community resources, establishing connections with relevant service providers.
  • Ensure timely processing of referrals to maintain patient flow.
  • Keep the primary care chart updated with information regarding specialist consultations, hospital admissions, and emergency visits.
  • Perform additional duties as required.
Required Skills and Qualifications:
  • Strong verbal and written communication abilities.
  • Exceptional customer service orientation.
  • Excellent organizational skills with a keen attention to detail.
  • Proficiency in Microsoft Office Suite or similar software.
  • Bilingual proficiency in English and Spanish is preferred.
Education and Experience:
  • A high school diploma or equivalent is mandatory. A minimum of one year of experience in referral management or a related medical office role is required.
  • Familiarity with electronic medical record (EMR) systems is advantageous.
Physical Requirements:
  • Ability to sit for extended periods while working on a computer.
  • Capability to lift items weighing up to 10 pounds occasionally.

Work Environment: This position is not remote and requires in-person attendance.

Job Type: Full-time

Compensation: Starting pay is $21.00 per hour, commensurate with experience.
