This position is PRN and will require working day shifts as needed.
Key Responsibilities:
- Deliver direct patient care, assess functional requirements and outcomes, and establish individualized care plans; evaluate treatment effectiveness and adjust as necessary. Ensure all therapy practices adhere to established standards of care.
- Maintain accurate documentation of evaluations, treatment objectives, and plans in line with hospital protocols and guidelines. Regularly update records and ensure timely management of prescriptions and signatures.
- Oversee, delegate, and supervise tasks assigned to support personnel.
- Collaborate with the interdisciplinary team regarding patient needs, treatment adjustments, and progress, including discharge planning.
- Foster a safe environment for patients, visitors, and staff. Identify safety risks and implement necessary precautions while adhering to approved safety and infection prevention standards.
- Be prepared to work during adverse weather conditions and staffing emergencies as required.
- A minimum of two years of experience in an inpatient hospital environment, with a preference for inpatient rehabilitation experience.
- Valid state license as a Physical Therapist in good standing is mandatory.
- Current BLS/CPR certification is required.
