We are excited to announce a career opportunity for a skilled Speech Language Pathologist to join our team at CommonSpirit Health at Home. About the RoleThe successful candidate will be responsible for providing home-based speech therapy services to patients, working closely with them and their families to develop and implement individualized treatment plans that promote effective communication and improve quality of life.
Responsibilities- Conduct thorough assessments and evaluations to identify patient needs and goals
- Develop and implement individualized speech therapy plans that address patient-specific challenges
- Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination
- Maintain accurate records and documentation of patient progress and outcomes
To be successful in this role, you will need:
- A Master's degree in Speech-Language Pathology from an accredited institution
- An ASHA certificate of clinical competence
- At least one year of clinical experience in speech therapy
