Job Summary

We are seeking a highly skilled and experienced Clinical Nurse Specialist to join our team. In this role, you will coordinate patient care across the continuum, collaborating with providers to identify needs for preventive care, evidence-based chronic disease management, and care coordination. Your expertise will include providing general and specific healthcare education, working directly with patients to identify and monitor self-management goals, and following up on ED visits and inpatient discharges.

The ideal candidate will have a strong background in nursing, excellent communication and interpersonal skills, and the ability to work collaboratively in small teams to improve operations. A bachelor's degree in nursing is preferred, but a graduate from an accredited nursing program is required. You must also possess a current RN or LPN license, as well as Basic Life Support certification.

Responsibilities
  • C Coordinate and facilitate quality clinical management of a broad-based outpatient population across the continuum of care.
  • P Collaborate with providers to identify needs for preventive care, evidence-based chronic disease management, and care coordination.
  • S Provide general and specific healthcare education, including educating patients on INR management and teaching for those on Coumadin using a standardized protocol.
  • A Assist providers with exams, treatments, and procedures as needed.
  • T Perform a direct patient care role, including nursing visits such as Annual Wellness Visits (AWVs) and Chronic Care Management (CCM) of BIDHC patients.
  • E Oversees the inventory and ordering management of clinical supplies and equipment, ensuring safe, accurate, and clinically appropriate collection, storage, and processing of all biologic specimens.
Requirements
  • D Graduate from an accredited Nursing Program required.
  • R Bachelor's degree preferred.
  • L Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required.
  • O Basic Life Support certification required.
Salary

$84,000 - $110,000 per year.
