Medically Focused Care Coordinator

Mass General Brigham is currently seeking a Registered Nurse for a Med Surg nursing position in Cambridge, Massachusetts. This is an exciting opportunity to work in a world-renowned hospital that provides exceptional care to patients.
About the JobThe ideal candidate will have experience in ER/community health center urgent care/medical surgical unit as a charge nurse. A background in pediatric and underserved populations would be beneficial.
This role will involve providing proficient, compassionate nursing care to patients and their loved ones. Responsibilities include:
- Collaborating with the Nursing Director and Patient Care Team to achieve efficient patient outcomes.
- Working with the patient towards efficient and effective personnel and unit management.
- Triage for all patients speaking publicly to the health center through urgent care.
- Implementing psycho-social assessment tools as part of the Primary Care/Behavioral health integration team.
- Provides and maintains a therapeutic environment for patients and families.
- Articulates the primary nursing role to the patient, family, and health team members.
- Identifies, facilitates, and evaluates the outcomes of nursing care for an individual patient or group of patients.
- Coordinates involvement of the patient, family, and health team members in patient care, including patient/family teaching and discharge planning.
- Current licensure as a registered nurse in the Commonwealth of Massachusetts.
- Experience preferred in ER/community health center urgent care/medical surgical unit as a charge nurse.
- CPR Certification required.
Estimated Salary: $83,500 - $124,000 per year based on location and experience.
