Job Overview

Easterseals Blake Foundation, a leading provider of comprehensive behavioral health services and support for individuals with intellectual and developmental disabilities, is seeking a dedicated individual to fill the role of Supported Living Program Coordinator. This position is pivotal in enhancing the quality of life for those we serve, ensuring they receive the necessary support in community-based settings.

Position Summary:

The Program Coordinator will report directly to the Associate Director and will oversee the daily operations of the Supported Living Program. This role includes supervising staff, providing direct care, and managing administrative responsibilities.

Key Responsibilities:

  • Oversee the recruitment, training, and performance management of staff, ensuring compliance with all training requirements.
  • Coordinate employee schedules while providing direct support to individuals in the program.
  • Monitor and support staff in developing community engagement activities.
  • Ensure adherence to all contractual obligations and maintain accurate documentation for each individual served.
  • Foster positive relationships with families, staff, and external professionals to promote effective communication and problem-solving.
  • Manage administrative tasks including billing, attendance tracking, and maintaining up-to-date member files.
  • Conduct regular audits of personal funds accounts and oversee payroll processes.
  • Identify potential participants for the Supported Living Program through evaluations and home visits.
  • Ensure confidentiality and security of client information at all times.
  • Administer medication and manage health-related tasks as required.
  • Facilitate regular staff meetings and participate in team discussions.
  • Promote a safe and healthy work environment for all staff and individuals served.

Qualifications:

  • Must possess a valid driver's license and meet agency driving standards.
  • Minimum age requirement of 21 years.
  • Experience working with individuals with intellectual and developmental disabilities is preferred.
  • Flexibility to work various shifts, including evenings and weekends.
  • Current certifications in CPR, First Aid, and Level One Clearance are required.
  • Ability to pass a background check and maintain a clean record.

  • Ability to perform physical tasks including lifting and transferring individuals as needed.
  • Capacity to remain calm and supportive in high-stress situations.
  • Flexibility to adapt to changing work demands and diverse environments.
