Position Overview:
The Registered Nurse Care Coordinator plays a vital role in overseeing personal care services and ensuring high-quality support for consumers.

Key Responsibilities:
- Supervise personal care staff and manage the delivery of care services.
- Conduct home visits and perform comprehensive consumer assessments.
- Develop tailored care plans to meet individual needs.
- Evaluate the performance of personal care workers and provide guidance.

Qualifications:
- A minimum of one year of supervisory experience is required.
- Must be a graduate of an accredited nursing program.
- A valid Registered Nurse license is necessary in the location of employment.
- Possession of a valid Driver's License with a clean driving record.
