The RN Hospice Nursing Coordinator plays a vital role within our leadership team and is also an integral part of the direct care staff.

In this position, you will deliver essential hospice nursing care to our patients, ensuring the highest standards of comfort and quality of life.

Your responsibilities will include:

  • Collaborating with other healthcare professionals to guarantee optimal ongoing care for patients.
  • Supporting the admissions team to facilitate timely and quality admissions.
  • Providing hands-on clinical nursing care while interacting with patients, families, physicians, and interdisciplinary team members.
  • Assessing and coordinating a pain and symptom management program in collaboration with patients, families, and the interdisciplinary team.
  • Facilitating choices and ensuring dignity for patients facing end-of-life challenges.
  • Offering support and education to families in alignment with the hospice philosophy of care, addressing physical, psychological, spiritual, and financial needs.
  • Participating in interdisciplinary team meetings and updating the Plan of Care (POC) for assigned patients based on their needs.
  • Coordinating with primary care providers and medical directors to manage medication profiles effectively.
  • Ensuring that hospice provides medications relevant to disease processes and symptom management.
  • Communicating with team members to coordinate all services related to hospice care for assigned patients.
  • Providing guidance and supervision to CNAs and LPNs involved in patient care.
  • Participating in the performance evaluation of clinical team members.
  • Assisting with the orientation of clinical staff involved in patient care.
  • Upholding the Hospice Code of Ethics and working collaboratively as a team member.
  • Reporting changes in patient conditions that necessitate adjustments in care levels.
  • Utilizing necessary medical supplies and equipment effectively.
  • Building positive relationships with residents, families, community members, and staff.
  • Demonstrating basic computer skills and a willingness to learn company applications.
