Job Description

Community Health Plan of Washington is seeking a dedicated Care Manager to join our team. As a Care Manager, you will work closely with enrolled members to develop and implement Health Action Plans that promote improved quality of life, health, and engagement.

The primary responsibility of this role is to work with members over time to seek and use resources that match their needs, providing the six Health Home services of comprehensive care management, care coordination, health promotion, individual and family support, care transitions, and referrals to community and social support services.

To be successful in this role, you will possess a current license as a registered nurse, practical nurse, mental health counselor, or a master's or bachelor's degree in social work, psychology, social services, human services, or behavioral sciences. You will also have at least two years of related experience in social services, healthcare, or case management, preferably with fieldwork.

The ideal candidate will have experience working with people with mental illness and/or substance use, as well as two years' experience with Medicare, Medicaid, and NCQA. You will also have the ability to travel to meet members where they reside, periodically for trainings, and meetings.

Key Responsibilities:

  • Engage prospective Health Homes participants through referrals received by Health Home Lead.
  • Manage a caseload of 50 members by taking initiative with managing the needs of the caseload, scheduling, and organizing time.
  • Conduct telephonic and face-to-face outreach and coordination of care activities for Health Homes members.
  • Coordinate and oversee the Health Home benefit by actively engaging the member via completion of Health Action Plan and supporting the member to achieve their short-term and long-term goals.
  • Improve continuity of care by helping members navigate relationships with providers and outside resources.

Requirements:

  • Current license as a registered nurse, practical nurse, mental health counselor, or a master's or bachelor's degree in social work, psychology, social services, human services, or behavioral sciences.
