Job Summary

We are seeking a skilled and compassionate Advanced Practice Clinician to join our team at Optum Home & Community Care. As a key member of our Patient Connect program, you will play a vital role in ensuring a smooth transition from rehab to home for patients who have recently been hospitalized.

Your primary responsibilities will include performing comprehensive assessments, documenting findings, and communicating with patients and healthcare teams to ensure seamless care coordination. You will also have the opportunity to work independently, using your expertise and experience to provide guidance and support to patients and families.

Key Responsibilities:
  • Perform comprehensive assessments and document findings in a concise and compliant manner
  • Ensure accurate and timely documentation of diagnoses and quality elements
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Travel between care sites as needed to provide patient care
  • Participate in quality initiatives and contribute to program enhancement
Requirements:
  • Certified Nurse Practitioner or Physician Assistant through a national board
  • Graduate of an accredited master's degree in Nursing or Physician Assistant degree program
  • Active and unrestricted license in the state of practice
  • Current active DEA licensure or ability to obtain post-hire
  • Access to reliable transportation and ability to lift 30 pounds
  • Availability to work 24 hours per month, with flexibility to work off-hours as needed
Preferred Qualifications:
  • 1+ years of hands-on post-grad experience in Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills and ability to document medical information
  • Ability to develop and maintain positive customer relationships
