POSITION SUMMARY

This role involves a variety of responsibilities aimed at assisting adults with intellectual and developmental disabilities in exploring retirement options and enhancing community participation. Responsibilities include providing transportation services using agency vehicles and personal vehicles, facilitating individuals' transport to and from their residences, which may extend the workday.

KEY RESPONSIBILITIES

The following duties are essential to this position, and additional tasks may be assigned:

  • Collaborate with an interdisciplinary team to support individuals in making informed choices regarding their social and recreational aspirations while addressing medical needs.
  • Encourage active participation of individuals in all stages of program development and execution, providing necessary training and support within designated timeframes.
  • Serve as the primary facilitator for individuals, promoting their engagement and integration within the community.
  • Establish and nurture connections with senior services and recreational personnel to enhance inclusion opportunities for individuals.
  • Assist individuals in accessing community services and resources.
  • Evaluate the physical environment of service locations to identify potential hazards and report safety concerns to the appropriate personnel.
  • Attend and support community-based recreational activities, including providing transportation for individuals.
  • Adhere to agency policies regarding the management of individual funds and documentation of expenses.
  • Utilize approved behavioral management techniques to enhance individuals' community integration.
  • Document individual progress and challenges in compliance with agency, state, and federal standards.
  • Provide disability awareness information to advocate for accessible community resources.
  • Assist individuals with personal care needs as necessary.

TRANSPORTATION DUTIES

Responsibilities include:

  • Driving agency-owned or personal vehicles to transport individuals to various activities.
  • Conducting pre- and post-trip inspections of vehicles, documenting findings and reporting any maintenance or safety issues.
  • Assisting passengers with mobility needs and ensuring their safety during transport.
  • Maintaining accurate records of trips and passenger behavior.

QUALIFICATIONS

To be successful in this role, candidates should possess:

  • A High School Diploma or G.E.D. with one to three years of relevant experience, or a bachelor's degree in a related field.
  • Strong communication skills, both verbal and written, to effectively convey information and document individual progress.
  • The ability to perform basic mathematical calculations.
  • Problem-solving skills to address practical challenges and advocate for individuals.
  • A valid driver's license in good standing and the ability to secure a Public Service License within a specified timeframe.

PHYSICAL AND WORK ENVIRONMENT REQUIREMENTS

This position requires the ability to perform physical tasks, including lifting and assisting individuals with mobility needs. The work environment may involve exposure to various weather conditions and moderate noise levels.
