About the Job

In this dynamic role, you will work closely with enrollees, their providers, and involved state agencies to ensure seamless communication and effective resource utilization. You will also have the opportunity to develop your skills and expertise while making a meaningful difference in the lives of others.

Key Responsibilities
  • Provide person-centered and recovery-oriented care management and coordination to support enrollees' health and well-being.
  • Foster collaboration among care teams and coordinate communication to achieve optimal outcomes.
  • Support enrollees in accessing and utilizing resources to improve their health and quality of life.
Requirements
  • Bachelor's degree in a related field, with a focus in psychology highly preferred.
  • 1-2 years of human service experience working with similar populations, or equivalent experience in a relevant field.
  • Valid driver's license and reliable vehicle access required.
  • Bilingual skills highly desirable, with language proficiency including Spanish, American Sign Language, and Vietnamese.
Salary and Benefits

We offer a competitive salary of $62,500 per annum, plus a $5,000 language differential bonus for bilingual candidates. Our comprehensive benefits package includes paid time off, health and dental insurance, employer-paid life insurance, 403(b) retirement plan, and more.
