Mental Health Support Specialist

North Suffolk Community Services Inc. is seeking a dedicated and empathetic Mental Health Support Specialist to join our Behavioral Health Community Partner (BH CP) program. As a key member of our team, you will play a vital role in supporting the health, safety, and quality of life of clients through advocacy, teaching, and service planning.
Your primary responsibilities will include providing person-centered and recovery-oriented care management and coordination, conducting outreach and engagement with enrollees, assisting them to identify goals and develop individual care plans, and facilitating care teams. You will also be responsible for completing comprehensive assessments, coordinating communication among enrollees, their providers, and other stakeholders, and supporting enrollees to access and utilize resources to improve and maintain their health and quality of life.
To be successful in this role, you will need a Bachelor's degree in a related field, preferably with a focus in psychology. 1-2 years of human service experience with a similar population is preferred. Valid driver's license and access to a reliable vehicle are required. Bilingual skills, particularly in Spanish, American Sign Language, or Vietnamese, are highly valued and may qualify for an additional salary differential.
We offer a competitive salary of $50,000 per year, plus a $5,000 language differential bonus for eligible candidates. Our comprehensive benefits package includes paid time off, health and dental insurance, retirement plan, disability insurance, and more.
