Job Description: Mental Health Support Specialist

Job Summary: We are seeking a dedicated Mental Health Support Specialist to join our Behavioral Health Community Partner (BH CP) program. The ideal candidate will have a passion for mental health advocacy and experience working with diverse populations.

About the Role:

This is a full-time position that involves providing person-centered care management and coordination to clients. You will work collaboratively with the team to develop individualized care plans, facilitate care teams, and coordinate communication among clients, providers, and state agencies.

Key Responsibilities:

  • Outreach and engagement with enrollees using telehealth and community visits
  • Assist enrollees to identify goals and develop an individual care plan
  • Complete comprehensive assessments
  • Facilitate care teams
  • Coordinate communication among enrollees, their providers, involved state agencies, and others

Requirements:

To be successful in this role, you must have a Bachelor's degree in a related field, preferably with a focus in psychology. You should also have 1-2 years of human service experience working with similar populations. Bilingual skills are highly valued, and you may be eligible for a language differential bonus.

Estimated Salary: $65,000 - $75,000 per year based on location and experience
