North Suffolk Community Services Inc is committed to providing high-quality behavioral health services to our clients. We are seeking a dedicated Care Coordinator to join our team and support our mission.

The successful candidate will work closely with clients, providers, and other stakeholders to ensure seamless care coordination and delivery. This is an excellent opportunity to make a meaningful difference in the lives of our clients.

Responsibilities
  1. Outreach and Engagement: Provide targeted outreach and engagement services to clients, using a combination of telehealth and community visits.
  2. Care Planning: Assist clients in identifying goals and developing individual care plans, in collaboration with their providers and other stakeholders.
  3. Assessment and Coordination: Conduct comprehensive assessments and facilitate care teams, ensuring effective communication among all parties involved.
Benefits
  • Competitive salary of $65,000 per year.
  • $5,000 language differential bonus for bilingual candidates.
