Key Responsibilities:

  • Conducts skilled nursing procedures.
  • Regularly assesses patients and their evolving conditions, focusing on physical, functional, psychosocial, and environmental needs, as well as daily living activities.
  • Responsible for documenting patient information in the electronic medical record system.
  • Ensures accurate documentation following the standards set by Community Hospice.
  • Delivers patient care according to physician directives and the goals set by patients and caregivers, including post-mortem care as per established protocols.
  • Creates and oversees the patient Plan of Care, supervising hospice aides in delivering care.
  • Maintains awareness of and adheres to Standard Precautions.
  • Provides emotional support to patients and caregivers.
  • Educates patients and caregivers as needed, in alignment with the care plan.
  • Demonstrates knowledge of medications and their administration.
  • Evaluates patients for the necessity of crisis intervention care and manages cases on a daily basis.
  • Conducts supervisory visits for Hospice Aides at least every two weeks to ensure patient and caregiver satisfaction with services provided.
  • Completes annual competency assessments of skills as per organizational standards.
  • Consults with the Community Hospice Medical Director or Associate Medical Director for recommendations on pain and symptom management, communicating these to patients, caregivers, and attending physicians for approval.
  • Communicates effectively and professionally with interdisciplinary team members, patients, and caregivers.
  • Actively engages the interdisciplinary team in the initiation, review, and evaluation of the Plan of Care.
  • Assesses services delivered to patients, informing the Manager or RN Supervisor of any feedback or opportunities for improvement.
  • Participates in interdisciplinary group meetings.
  • Facilitates patient transfers to inpatient facilities, ensuring the ordering of medical equipment, supplies, and medications according to organizational procedures, while providing Continuous/Transitional Care during transfers.
  • Informs the Manager or RN Supervisor of necessary medical supplies, equipment, and medications as directed.
  • Ensures that documentation reflects the care provided, patient status, and any concerns noted, maintaining clarity and thoroughness.
  • Keeps Care Plans updated and relevant to patient and caregiver needs.
  • Completes Patient Incident Reports accurately and submits them promptly to the Manager or RN Supervisor.
  • Maintains operational systems in compliance with Federal and State hospice regulations.
  • Adheres to Community Hospice Pharmacy and State regulations regarding pharmaceuticals.
  • Takes initiative in identifying and addressing operational needs, communicating with the appropriate departments for resolution.
  • Fosters professional relationships with both internal and external stakeholders.
  • Promotes the organization's image within the community.
  • Utilizes the chain of command for effective problem-solving.
  • Exhibits appropriate professional appearance and grooming.
  • Demonstrates courtesy, patience, and respect in all interactions.
  • Maintains a reliable attendance record, notifying the organization of any absences as per policy.
  • Engages in organizational Quality Assurance Performance Improvement (QAPI) initiatives as applicable.
  • Adheres to and supports established Clinical Care standards.

Qualifications:

Education and Experience:

Any combination of education and experience that equips the individual with the necessary skills and knowledge for successful performance is acceptable. Typical qualifications include:

  • A degree from an accredited nursing program, either a two-year Associate Degree or a Bachelor's Degree in Nursing.
  • One year of experience as a Registered Nurse, with preference given to those with one year of medical-surgical or oncology experience in a hospital setting, or
  • Five years of experience as a Licensed Practical Nurse within the last five years, including one year of medical-surgical or oncology experience in a hospital setting.
  • Understanding of educational principles and methods relevant to hospice nursing.
  • Strong communication and teamwork skills.
  • Ability to confidently manage inpatient facility operations and patient care needs during shifts.
  • Willingness to work long and occasionally irregular hours.
