Responsibilities
The RN Case Manager is responsible for:
- Coordinating the overall interdisciplinary plan of care for patients.
- Developing care plans that reflect patient and family preferences and values.
- Monitoring patient care through assessments and evaluations.
- Advocating for necessary resources and addressing barriers to care.
- Maintaining ongoing communication with supervisors and healthcare providers to ensure effective implementation of care plans.
- Serving as a resource for medical record documentation and appropriateness of services related to diagnoses and post-discharge care options.
Requirements
Licensure / Certification / Registration:
- Registered Nurse credentialed from the Florida Board of Nursing required prior to hire or transfer.
- BLS Provider certification preferred, with American Heart Association or American Red Cross accepted.
Education:
- Diploma from an accredited nursing school or required professional licensure at the time of hire.
