- Conducts advanced nursing interventions.
- Regularly assesses patients and their evolving needs, including physical, functional, psychosocial, and environmental aspects.
- Responsible for entering patient data into the electronic health record system.
- Ensures documentation aligns with Community Hospice standards.
- Delivers patient care as per physician directives and patient/caregiver objectives, including post-mortem care as per established protocols.
- Creates and oversees the patient Plan of Care, supervising hospice aides in the delivery of care.
- Maintains awareness of and adheres to Standard Precautions.
- Offers emotional support to patients and caregivers.
- Educates patients and caregivers as necessary based on the care plan.
- Demonstrates knowledge and proper administration of medications.
- Evaluates patients for the necessity of crisis intervention and oversees daily case management.
- Conducts supervisory visits for Hospice Aides every 14 days to ensure patient/caregiver satisfaction with services.
- Completes annual on-site evaluations of aides during patient care.
- Maintains competency in required skills, completing annual assessments as per organizational standards.
- Communicates with the Community Hospice Medical Director or Associate Medical Director for recommendations on pain and symptom management.
- Engages in timely and professional communication with interdisciplinary team members, patients, and caregivers.
- Involves the interdisciplinary team in the initiation, review, and evaluation of the Plan of Care.
- Reviews services provided to patients and informs management of any feedback or areas for improvement.
- Participates in interdisciplinary group meetings.
- Facilitates patient transfers to inpatient facilities, coordinating medical equipment, supplies, and medications as per organizational procedures.
- Informs management of required medical supplies and medications as directed.
- Ensures documentation accurately reflects patient care, services provided, and any noted concerns.
- Keeps Care Plans updated and relevant to patient and caregiver needs.
- Accurately completes and submits Patient Incident Reports to management.
- Adheres to Federal and State hospice regulations.
- Complies with Community Hospice Pharmacy and State guidelines regarding pharmaceuticals.
- Takes initiative in identifying and addressing operational needs within the facility.
- Maintains professional relationships with both internal and external stakeholders.
- Promotes the organization's image within the community.
- Utilizes the chain of command for problem resolution.
- Exhibits appropriate professional appearance and grooming.
- Demonstrates courtesy, patience, and respect in all interactions.
- Maintains a reliable attendance record and notifies the organization of any absences in accordance with policy.
- Engages in Quality Assurance Performance Improvement (QAPI) initiatives as appropriate.
- Adheres to established Clinical Care standards.
Qualifications:
Education and Experience:
Any combination of education and experience that equips the individual with the necessary skills and knowledge for effective performance is acceptable. Typical qualifications include:
- A degree from an accredited nursing program, either a 2-year Associate Degree or a Bachelor's Degree in Nursing.
- One year of experience as a Registered Nurse, with preference for experience in hospital medical-surgical or oncology settings, or
- Five years of experience as a Licensed Practical Nurse within the last five years, including one year in a hospital medical-surgical or oncology environment.
- Understanding of educational principles and methods relevant to hospice nursing.
- Strong communication and teamwork skills.
- Ability to manage inpatient facility operations and patient care requirements effectively.
- Willingness to work long and occasionally irregular hours.
