Key Responsibilities:

  • Conducts skilled nursing interventions.
  • Regularly assesses patients' conditions, including physical, functional, psychosocial, and environmental needs, as well as daily living activities.
  • Responsible for documenting patient information in the electronic medical record system.
  • Ensures documentation aligns with Community Hospice standards.
  • Delivers patient care following physician directives and patient/caregiver objectives as outlined in the Care Plan; provides post-mortem care as per established protocols.
  • Creates and manages the patient Plan of Care while overseeing hospice aides in delivering care.
  • Maintains awareness of and adheres to Standard Precautions.
  • Offers emotional support to patients and caregivers.
  • Educates patients and caregivers as relevant to their situation and care plan.
  • Demonstrates knowledge of medications and administers them appropriately.
  • Evaluates patients for the necessity of crisis intervention and supervises cases on a daily basis.
  • Conducts supervisory visits for Hospice Aides at least every 14 days to gauge patient and caregiver satisfaction with services.
  • Completes annual on-site evaluations of aides while they provide care.
  • Maintains competency in identified skills and completes annual competency assessments as per organizational standards.
  • Consults with the Community Hospice Medical Director or Associate Medical Director for recommendations on pain and symptom management, communicating these to patients, caregivers, and attending physicians as necessary.
  • Communicates effectively and professionally with interdisciplinary team members, patients, caregivers, and other departments within Community Hospice.
  • Engages the interdisciplinary team in the initiation, review, and evaluation of the Plan of Care.
  • Assesses services provided to patients and informs the Manager or RN Supervisor of any feedback or areas for improvement.
  • Participates in interdisciplinary group meetings.
  • Facilitates patient transfers to inpatient facilities, coordinating medical equipment, supplies, and medications in accordance with organizational procedures; ensures continuity of care during transitions.
  • Informs the Manager or RN Supervisor of required medical supplies, equipment, and medications as directed.
  • Ensures documentation accurately reflects patient care, services rendered, and any concerns, maintaining legibility and thoroughness.
  • Keeps Care Plans updated and relevant to patient and caregiver needs.
  • Completes Patient Incident Reports accurately and submits them promptly to the Manager or RN Supervisor.
  • Adheres to operational systems in compliance with Federal and State hospice regulations.
  • Follows Community Hospice Pharmacy and State regulations regarding pharmaceuticals.
  • Takes initiative in addressing operational needs and advises the appropriate department for resolution.
  • Maintains professional relationships with both internal and external stakeholders.
  • Promotes the organization's image within the community.
  • Utilizes the chain of command for problem resolution.
  • Exhibits appropriate professional attire and grooming.
  • Demonstrates courtesy, patience, and respect in all interactions.
  • Maintains a reliable attendance record and adheres to organizational policies regarding absences.
  • Participates in organizational Quality Assurance Performance Improvement (QAPI) initiatives as appropriate.
  • Supports and adheres to established Clinical Care standards.

Qualifications:

Education and Experience:

Any combination of education and experience that equips the candidate with the necessary skills and knowledge for effective performance is acceptable. Typical qualifications include:

  • A degree from an accredited nursing program, either a 2-year Associate Degree or a Bachelor's Degree in Nursing.
  • One year of experience as a Registered Nurse, preferably with a background in hospital medical-surgical or oncology, or
  • Five years of experience as a Licensed Practical Nurse within the last five years, including one year in a hospital medical-surgical or oncology setting.
  • Understanding of educational principles and methods relevant to hospice nursing.
  • Strong communication and teamwork skills.
  • Ability to manage inpatient facility operations and patient care requirements effectively during shifts.
  • Willingness to work long and occasionally irregular hours.
