Clinical Pharmacist II- Per Diem

We are seeking a highly skilled Clinical Pharmacist II to join our team at NorthBay Health. As a Clinical Pharmacist II, you will play a critical role in providing pharmaceutical care to patients, ensuring that they receive the highest level of care consistent with the goals of our Department and Organization.
Responsibilities- Provide clinical judgment and professional knowledge in the Pharmacy Department to ensure that patients receive pharmaceutical care that meets the highest standards.
- Develop and implement pharmaceutical care plans that are tailored to the needs of individual patients.
- Collaborate with healthcare teams to ensure that patients receive comprehensive care that addresses their medical, emotional, and social needs.
- Stay up-to-date with the latest developments in pharmacy practice and apply this knowledge to improve patient outcomes.
- Participate in quality improvement initiatives to ensure that our pharmacy services meet the highest standards of quality and safety.
- Doctor of Pharmacy (Pharm D.) or Bachelor of Science Degree in Pharmacy.
- California Board of Pharmacy licensure.
- One year of hospital-based experience in unit-dose distribution and IV additives and/or hospital residency.
- Excellent verbal and written communication skills.
- Ability to apply professional knowledge and judgment to the provision of pharmaceutical care for patients of all ages.
- Competitive hourly salary range of $85.05-$98.50, with opportunities for advancement based on experience.
- Hourly shift differentials for PM and NOC shifts.
