About This Nursing Position

HopeHealth is seeking a Care Navigator Registered Nurse (RN) who will be the primary HopeHealth representative in a designated territory, collaborating in the transition of patients throughout HopeHealth's continuum of care, which includes home health, palliative care, and hospice services.

Key Responsibilities:
  • Represent HopeHealth Service Lines: Represent HopeHealth service lines (home health, palliative care, and hospice) in assigned hospitals, nursing homes, assisted living facilities, physicians' offices, health centers, and the community at large.
  • Cultivate and Develop Relationships: Cultivate and develop relationships and drive growth in assigned hospitals, nursing homes, assisted living facilities, physician offices, health centers, and the community at large.
  • Work in Collaboration: Work in close collaboration with home health, palliative care, hospice, and the Access Center to provide best-in-class customer service and transition patients to the appropriate level of care.
  • Collaborate with Customers and Internal Teams: Collaborate with customers and internal teams in the effective discharge of patients to the appropriate service line.
  • Provide Patient and Family Information Sessions: Provide family and patient information sessions on home health, palliative care, and hospice services.
  • Complete Goals of Care Conversations and Hospice Evaluations: Complete goals of care conversations and hospice evaluations.
  • Submit Accurate Documentation: Submit accurate, relevant, and timely documentation in the EMR as appropriate.
  • Arrange Care Coordination Meetings: Arrange care coordination meetings and follow up on service recovery issues.
  • Utilize CRM: Leverage CRM to identify opportunities for growth and document daily territory activities and results on an ongoing basis.
  • Provide Educational Sessions: Provide and present educational sessions on home health, palliative care, and hospice care services.
  • Attend Professional Meetings: Seek out and attend professional meetings to further develop relationships and drive census growth.
  • Apply HopeHealth Values: Consistently apply HopeHealth values in all aspects of work, complying with established mission, policies, procedures, and objectives in quality assurance, safety, environment, infection control, and HIPAA compliance.
  • Complete Other Duties: Complete other duties as requested by the manager.
Qualifications:

  • Registered Nurse Required: Registered Nurse required; BSN preferred.
  • RN License Required: RN license required in RI and MA.
  • Minimum of 5 Years of Clinical Experience: Minimum of 5 plus years of clinical experience.
  • Excellent Nursing Assessment and Clinical Skills: Excellent nursing assessment, clinical, and communication skills.
  • Home Care and/or Hospice Experience: Home care and/or hospice preferred.
  • Valid Driver's License: Valid drivers license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.
  • Ability to Read and Analyze Professional Journals: Ability to read, analyze, and interpret professional journals, basic financial reports, medical records, and legal documentation skills.
  • Proficient Skills Using a Computer: Proficient skills using a computer are mandatory.
  • Critical Thinking Skills: Critical thinking skills inherent to the position. Ability to evaluate situations according to established standards and policies.
