About This Nursing Position

HopeHealth is seeking a Care Navigator Registered Nurse (RN) who will be the primary HopeHealth representative in a designated territory, collaborating in the transition of patients throughout HopeHealth's continuum of care, which includes home health, palliative care, and hospice services.

The role is responsible for driving growth and reputation for HopeHealth's service lines.

Key Responsibilities
  • Represent HopeHealth Service Lines
    • Represents HopeHealth service lines (home health, palliative care, and hospice) in assigned hospitals, nursing homes, assisted living facilities, physician's offices, health centers, and the community at large.
  • Cultivate and Develop Relationships
    • Cultivates and develops relationships and drives growth in assigned hospitals, nursing homes, assisted living facilities, physician offices, health centers, and the community at large.
  • Work in Collaboration
    • Works in close collaboration with home health, palliative care, hospice, and the Access Center to provide best-in-class customer service and transition patients to the appropriate level of care.
  • Collaborate with Customers and Internal Teams
    • Collaborates with customers and internal teams in the effective discharge of patients to the appropriate service line.
  • Provide Patient and Family Information Sessions
    • Provides family and patient information sessions on home health, palliative care, and hospice services.
  • Complete Goals of Care Conversations and Hospice Evaluations
    • Completes goals of care conversations and hospice evaluations.
  • Submit Accurate Documentation
    • Submits accurate, relevant, and timely documentation in the EMR as appropriate.
  • Arrange Care Coordination Meetings
    • Arranges care coordination meetings and follows up on service recovery issues.
  • Utilize CRM to Identify Opportunities
    • Leverages CRM to identify opportunities for growth and documents daily territory activities and results on an ongoing basis.
  • Provide Educational Sessions
    • Provides and presents educational sessions on home health, palliative care, and hospice care services.
  • Attend Professional Meetings
    • Seeks out and attends professional meetings to further develop relationships and drive census growth.
  • Apply HopeHealth Values
    • Consistently applies HopeHealth values in all aspects of work, complying with established mission, policies, procedures, and objectives in quality assurance, safety, environment, infection control, and HIPAA compliance.
  • Complete Other Duties
    • Completes other duties as requested by the manager.
Qualifications

  • Registered Nurse Required
    • Registered Nurse required; BSN preferred
  • License and Experience
    • RN license required in RI and MA
    • Minimum of 5 plus years of clinical experience
  • Excellent Nursing Assessment and Clinical Skills
    • Excellent nursing assessment, clinical, and communication skills
  • Home Care and/or Hospice Experience
    • Home care and/or hospice preferred
  • Valid Driver's License
    • Valid drivers license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order
  • Ability to Read and Analyze
    • Ability to read, analyze, and interpret professional journals, basic financial reports, medical records, and legal documentation skills. Communicate professionally both verbally and in writing.
  • Computer Proficiency
    • Proficient skills using a computer are mandatory.
  • Critical Thinking Skills
    • Critical thinking skills inherent to the position. Ability to evaluate situations according to established standards and policies.
