Job Summary

LCH Health and Community Services is seeking a highly skilled Registered Nurse to join our team as a Chronic Care Specialist. This is a challenging and rewarding role that requires a strong commitment to providing high-quality patient care.

Key Responsibilities
  • Chronic Care Management: Provide comprehensive care management to high-risk patients, including triage, medication management, and education on disease prevention and management.
  • Team-Based Care: Collaborate with a multidisciplinary team of healthcare professionals to provide patient-centered care, including motivational interviewing, healthcare coaching, and population health management.
  • Leadership and Supervision: Supervise and manage unlicensed staff, ensuring that they are providing high-quality care to patients.
  • Travel and Flexibility: May be required to travel between LCH's three locations, providing flexibility and adaptability in a fast-paced environment.
Requirements
  • Bilingual Proficiency: Must be proficient in English and Spanish, with the ability to communicate effectively with patients and families.
  • Education and Licensure: Must be a graduate of an accredited School of Nursing with a bachelor's degree in Nursing, and hold a current RN license in the Commonwealth of PA.
  • Certifications: Must hold current CPR certification.
